Instructions:

1. Before starting this survey, please print out a copy of the Tally Sheet. It is needed to record the answers to the survey.
   
   
2. Answer the questions honestly and not according to what you think are the right answers or what you think you should say.
   
   
3. The survey questions consist of 6 rows of 17 columns each.

5. When you have completed the questions, total your answers in each category(column) and list the three talents that are strongest within you.
   
   
6. A total of 20-25 in any category is a strong indication of a talent.
   
   
7. Oftentimes, people have more than one talent and it is in this combination of talents that a ministry will be found.
